• In case of lost card the replacement card can take up to 3 days to get activated. Until that duration no discount will be entertained.
• Carry this card at the time of hospital visit, no photograph of the card will be considered.
• This card is only valid for the card holder and his/her dependents, the dependents need to carry the card at the hospital.
• Free home sample collection up to 10km and use wallet money on it
